Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, focusing on dignity and comfort
- Administer medications and treatments safely and accurately according to orders
- Perform thorough patient assessments and document changes in condition promptly
- Collaborate with interdisciplinary team members to create and follow individualized care plans
- Support rehabilitation activities, assist with ADLs, and encourage resident mobility and independence
- Communicate clearly and kindly with residents’ families and contribute to care conferences
- Maintain accurate, timely documentation in the resident chart and electronic health record
Qualifications
- Current, active Massachusetts LPN license (candidates who are RNs are also welcome to apply)
- Current BLS/CPR certification required; ACLS or other certifications a plus
- Comfortable working 12-hour day shifts and supporting a rehab nursing population
- Ability to pass required background checks and health screenings
- Strong interpersonal skills and a patient-centered approach to care
Education
- Graduation from an accredited practical nursing program (LPN)
- High school diploma or GED required; additional nursing education (ADN/BSN) welcomed for RN applicants
Knowledge & Skills
- Solid knowledge of medication administration and safe nursing practices
- Familiarity with rehabilitation nursing principles and post-acute care needs
- Comfortable with electronic medical records and clear clinical documentation
- Good assessment skills and the ability to prioritize care in a busy setting
- Team player mindset and effective communication with residents, families, and staff
