Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled rehab and nursing setting
• Perform thorough assessments and document changes in condition clearly and promptly
• Administer medications and treatments safely and according to facility policy
• Collaborate with interdisciplinary team members to develop and update care plans
• Support rehabilitation goals by assisting with activities of daily living and therapeutic programs
• Communicate effectively with residents, families, and staff to ensure high-quality care
• Maintain accurate clinical documentation and follow infection control practices

Qualifications

• Active LPN or RN license (Licensed Practical Nurse or Registered Nurse) — Massachusetts license preferred
• Previous experience in a skilled nursing, rehabilitation, or long-term care setting is a plus
• Current BLS/CPR certification preferred or required by facility
• Strong interpersonal skills and the ability to work as part of a multidisciplinary team
• Demonstrated ability to perform assessments, administer medications, and document care accurately

Education

• Graduate of an accredited practical nursing program (LPN) or accredited nursing program (ADN/BSN for RN)
• Facility may require proof of education and state licensure

Knowledge & Skills

• Clinical assessment and care planning
• Medication administration and safety practices
• Familiarity with rehab nursing principles and supporting ADLs
• Clear, compassionate communication with residents and families
• Basic charting and documentation skills (electronic or paper records)
• Teamwork, time management, and prioritization in a fast-paced setting
