Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and document thoroughly
• Perform focused assessments and monitor changes in resident condition
• Collaborate with nurses, therapists, and support staff to implement individualized care plans
• Communicate clearly and empathetically with residents and families about care goals
• Maintain accurate, timely charting in the facility’s record system
• Follow infection control protocols and safety guidelines to protect residents and staff
• Support admissions, transfers, and discharge processes to ensure smooth transitions

Qualifications

• Active Massachusetts LPN license (LPN) — or active RN license if applying as an RN (this posting supports both LPN and RN clinicians)
• Current BLS (Basic Life Support) certification
• Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM) and providing hands-on resident care
• Strong communication skills and a team-first attitude
• Ability to pass background checks and meet facility onboarding requirements

Education

• Completion of an accredited LPN program (or ADN/BSN for RNs)
• High school diploma or equivalent

Knowledge & Skills

• Patient-centered nursing care and assessment skills
• Medication administration and safe documentation practices
• Familiarity with electronic medical records and accurate charting
• Basic wound care and therapies commonly used in rehab/skilled nursing
• Effective communication with multidisciplinary teams and families
• Time management and the ability to prioritize in a fast-paced environment
