Roles & Responsibilities

• Provide compassionate daily care to residents, including bathing, grooming, and mobility assistance
• Administer medications and follow physician orders safely and accurately
• Perform routine patient assessments and document changes in condition
• Collaborate with RNs, therapists, and interdisciplinary team members to support care plans
• Assist with wound care and restorative nursing under standing orders
• Communicate clearly and compassionately with residents and families to promote trust
• Maintain accurate, timely charting in the facility EMR and follow infection control protocols

Qualifications

• Active LPN license in Massachusetts (required)
• Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
• Comfortable working in a long-term care or rehab setting; clinical experience preferred
• Strong communication skills and the ability to work as part of a caring team
• Current immunizations per facility policy and background clearances as required

Education

• Licensed Practical Nurse: LPN diploma or certificate from an accredited program
• Registered Nurse pathway: ADN or BSN if pursuing RN role (if applicable)

Knowledge & Skills

• Medication administration and documentation expertise
• Skilled in resident assessments and recognizing change in condition
• Familiarity with wound care, restorative nursing, and basic clinical procedures
• Proficient charting and comfort using electronic medical records
• Solid infection control practices and safe patient handling techniques
• Ability to multitask in a fast-paced, rewarding care environment
