Roles & Responsibilities
- Provide compassionate daily care to residents, including bathing, grooming, and mobility assistance
- Administer medications and follow physician orders safely and accurately
- Perform routine patient assessments and document changes in condition
- Collaborate with RNs, therapists, and interdisciplinary team members to support care plans
- Assist with wound care and restorative nursing under standing orders
- Communicate clearly and compassionately with residents and families to promote trust
- Maintain accurate, timely charting in the facility EMR and follow infection control protocols
Qualifications
- Active LPN license in Massachusetts (required)
- Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
- Comfortable working in a long-term care or rehab setting; clinical experience preferred
- Strong communication skills and the ability to work as part of a caring team
- Current immunizations per facility policy and background clearances as required
Education
- Licensed Practical Nurse: LPN diploma or certificate from an accredited program
- Registered Nurse pathway: ADN or BSN if pursuing RN role (if applicable)
Knowledge & Skills
- Medication administration and documentation expertise
- Skilled in resident assessments and recognizing change in condition
- Familiarity with wound care, restorative nursing, and basic clinical procedures
- Proficient charting and comfort using electronic medical records
- Solid infection control practices and safe patient handling techniques
- Ability to multitask in a fast-paced, rewarding care environment
