Roles & Responsibilities
- Provide compassionate direct patient care, helping residents feel safe and respected
- Administer medications and treatments accurately and on schedule
- Perform thorough patient assessments and report changes to the care team
- Maintain clear, timely, and accurate patient records and documentation
- Collaborate with nurses, therapists, and physicians to deliver coordinated care
- Assist with admissions, discharges, and transitions of care with empathy
- Support residents and families with education and compassionate communication
Qualifications
- Active LPN license in Massachusetts (required)
- Current BLS/CPR certification (required or obtain upon hire)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong interpersonal skills and ability to work as part of an interdisciplinary team
- Dependable, patient-focused, and able to manage clinical priorities in a busy setting
Education
- Completion of an accredited LPN/LVN program or equivalent vocational nursing diploma
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and restorative nursing support
- Patient assessment, vital signs, and clinical observation
- Familiarity with electronic medical records (EMR) or willingness to learn
- Excellent communication, time management, and prioritization skills
