Roles & Responsibilities
- Provide compassionate direct patient care and daily nursing tasks with professionalism
- Administer medications and treatments accurately and document them thoroughly
- Perform routine patient assessments and report changes promptly to the care team
- Collaborate with nurses, therapists, and physicians to create and follow effective care plans
- Maintain clear, timely electronic health record documentation and handoffs
- Support patient comfort, dignity, and safety while educating patients and families
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania
- Current Basic Life Support (BLS/CPR) certification (or willingness to obtain)
- Comfortable administering medications, performing patient assessments, and recording care
- Strong communication and teamwork skills with a patient-centered attitude
- Ability to pass facility background checks and health screenings
Education
- Completion of an accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Solid clinical judgment for routine patient assessments
- Medication administration and reconciliation best practices
- Comfortable with electronic health records and accurate charting
- Wound care basics and infection control principles
- Strong interpersonal skills and ability to work effectively in a multidisciplinary team
