Roles & Responsibilities

• Provide compassionate, hands-on nursing care and daily assessments for residents
• Administer medications and treatments accurately and document in the medical record
• Monitor changes in patient condition and communicate findings to the care team
• Collaborate with nurses, therapists, and providers to develop and implement care plans
• Support residents with activities of daily living while preserving dignity and independence
• Maintain accurate, timely charting and ensure compliance with facility policies
• Participate in shift handoffs and team communication to ensure safe continuity of care

Qualifications

• Current LPN or RN license in Massachusetts (or compact/state license as applicable)
• Minimum clinical experience preferred; new grads welcome to inquire about opportunities
• Current Basic Life Support (BLS) certification required or obtained upon hire
• Strong communication skills and ability to work as part of a multidisciplinary team
• Reliable, patient-centered approach with attention to detail and documentation
• Ability to pass background checks and required facility screenings

Education

• LPN: Graduate of an accredited practical nursing program or equivalent
• RN: Graduate of an Associate or Bachelor of Science in Nursing program as applicable

Knowledge & Skills

• Proficient in medication administration and wound care basics
• Comfortable with resident assessments and recognizing changes in condition
• Familiarity with electronic medical record documentation or willingness to learn
• Good time management, prioritization, and clinical decision-making skills
• Compassionate bedside manner and ability to support families and residents
• Team player who contributes to a positive, safety-focused culture
