Roles & Responsibilities

• Coordinate patient care and transitions across the hospital and post-acute settings to achieve safe, timely discharges
• Perform utilization review using InterQual criteria to ensure appropriate level of care
• Develop and document individualized plans of care in Epic, keeping charts accurate and timely
• Facilitate family meetings and collaborate with patients, families, and community partners to support discharge goals
• Arrange DME, SNF placement, and ambulance transport independently and efficiently
• Work closely with interdisciplinary teams including social services and transportation for comprehensive care coordination
• Apply knowledge of DRG and CMS regulations to support regulatory compliance and appropriate billing classification
• Respond to weekend rotations and department needs while maintaining high-quality case management

Qualifications

• Active California license required (pending license accepted but must be licensed by start date)
• Minimum 2 years of acute care hospital case management experience with demonstrated care coordination and discharge planning
• Experience with utilization review/InterQual criteria and computerized documentation (Epic required)
• Travel/contract experience preferred; first-time travelers can be screened and considered
• Comfort with weekend rotations (every other weekend) and department staffing needs
• Proficiency with Microsoft Office and electronic charting systems
• Knowledge or experience with DRG and CMS processes and expectations
