Location: San Rafael, CA

Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Implement treatment plans developed by the supervising Physical Therapist to help residents regain mobility and independence
Deliver therapy focused on mobility, strength, balance, and functional tasks with compassion and clinical skill
Guide residents through therapeutic exercises and activities, adapting approaches to individual abilities
Monitor patient progress closely and report meaningful changes to the supervising Physical Therapist
Maintain accurate, timely therapy documentation to support continuity of care
Collaborate with interdisciplinary care teams to create cohesive, person-centered recovery plans
Educate residents and families on safe mobility techniques and practical fall prevention strategies

Qualifications

Active California Physical Therapy Assistant (PTA) License
Associate Degree from an accredited PTA program
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong patient care and communication skills to connect with residents and families
Demonstrated ability to work effectively within a multidisciplinary healthcare team

Education

Associate Degree from an accredited Physical Therapy Assistant (PTA) program

Knowledge & Skills

Expertise in mobility training and rehabilitation program delivery
Strong observation and patient monitoring skills
Clear, compassionate communication and thorough documentation abilities
Ability to collaborate across nursing, therapy, and ancillary teams
