Roles & Responsibilities

Conduct comprehensive psychosocial and clinical assessments to understand patient needs and strengths
Develop individualized, evidence-based treatment plans that promote recovery and resilience
Deliver individual and family therapy across the lifespan using trauma-informed and evidence-based interventions
Coordinate care with primary care providers, nurses, and behavioral health team members to ensure whole-person support
Document clinical findings, progress notes, and treatment plans accurately and timely to support continuity of care
Provide crisis intervention and appropriate referrals for substance use, severe mental health needs, and community resources
Engage underserved and rural populations with culturally sensitive, compassionate care that addresses barriers to access

Qualifications

Active Licensed Independent Social Worker (LISW) license in Iowa (required)
Master's degree in Social Work from an accredited program (required)
Minimum of 2 years of clinical experience preferred, with experience in individual and family therapy
Experience in rural or medically underserved settings is a strong plus
Strong documentation, interpersonal, and organizational skills
Demonstrated ability to collaborate effectively within an integrated care team

Education

Master's degree in Social Work from an accredited program

Knowledge & Skills

Clinical assessment and psychosocial evaluation
Treatment planning and delivery of evidence-based therapeutic interventions
Care coordination and collaboration with primary care and nursing teams
Trauma-informed care, depression and anxiety treatment, and substance use interventions
Comfort serving underserved and rural populations with sensitivity and adaptability
Accurate clinical documentation and progress note completion
