Roles & Responsibilities

• Implement treatment plans developed by the supervising Occupational Therapist to help residents regain independence
• Deliver therapy focused on activities of daily living (ADLs), mobility, and functional tasks
• Guide residents through therapeutic exercises and functional activities with patience and encouragement
• Monitor resident progress and promptly report changes to the supervising Occupational Therapist
• Maintain accurate, timely therapy documentation to ensure continuity of care
• Collaborate with interdisciplinary care teams to create cohesive, person-centered care plans
• Educate residents and families on safe techniques and adaptive strategies for daily living

Qualifications

• Active California Certified Occupational Therapy Assistant (COTA) license required
• Associate Degree from an accredited COTA program required
• Previous SNF, rehabilitation, or geriatric therapy experience preferred
• Strong patient care focus with excellent communication skills
• Comfortable working within a multidisciplinary healthcare team

Education

• Associate Degree from an accredited COTA program

Knowledge & Skills

• Experience supporting ADL training and functional rehabilitation
• Strong observation and patient monitoring abilities
• Clear, professional documentation skills
• Effective communication with residents, families, and care teams
• Ability to follow treatment plans and adapt activities to resident needs
