Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and on schedule
- Perform thorough patient assessments and report changes promptly
- Document care clearly and timely in the electronic health record
- Collaborate with interdisciplinary team members to create and follow care plans
- Assist with activities of daily living while promoting patient dignity and independence
- Participate in shift handoffs and communicate important patient information
Qualifications
- Active Licensed Practical Nurse (LPN) license in Massachusetts
- Recent LPN clinical experience in long-term care, rehabilitation, or skilled nursing preferred
- Current BLS/CPR certification required
- Ability to pass background check and required health screenings
- Strong communication skills and ability to work as part of a multidisciplinary team
Education
- Graduate of an accredited LPN/Vocational Nursing program
- High school diploma or GED required
Knowledge & Skills
- Solid knowledge of medication administration and documentation practices
- Familiarity with wound care, vital signs monitoring, and ADL support
- Comfortable using electronic health records and basic computer skills
- Excellent patient-centered communication and empathy
- Time management and prioritization in a fast-paced care environment
- Ability to follow care plans and adapt to individual resident needs
