Roles & Responsibilities

• Provide compassionate, high-quality nursing care to patients as a Licensed Practical Nurse
• Administer medications and treatments accurately and on schedule
• Monitor patients’ conditions and report changes to the interdisciplinary team
• Document assessments, interventions and patient responses clearly in the chart
• Support patient comfort and basic needs, including activities of daily living
• Collaborate with RNs and other clinicians to develop and follow care plans
• Educate patients and families about medications, treatments and discharge instructions
• Maintain infection control standards and safety protocols to protect patients and staff

Qualifications

• Current, active LPN license (ability to practice in New Hampshire preferred)
• Minimum clinical experience as an LPN (travel experience a plus)
• Basic Life Support (BLS) certification required; ACLS/PALS as applicable a plus
• Reliable, professional attitude with strong communication and teamwork skills
• Ability to complete required facility onboarding and health screenings

Education

• Graduate of an accredited LPN program or comparable vocational nursing program

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with basic patient assessments and monitoring
• Familiarity with wound care and common nursing procedures
• Ability to work in an electronic health record (EHR) environment
• Good time management, prioritization and delegation when needed
• Compassionate bedside manner and effective patient/family education
