Roles & Responsibilities
- Provide skilled physical therapy assistant services in patients' homes, delivering compassionate, hands-on care
- Conduct OASIS assessments and document accurate clinical notes using EPIC (remote client) and Rover mobile charting
- Manage a focused caseload (may visit up to 6 patients per day) while maintaining productivity expectations
- Develop and implement individualized treatment plans in collaboration with the therapy team
- Educate patients and caregivers on safety, mobility, and home exercise programs to promote independence
- Coordinate with interdisciplinary team members and refer when appropriate to ensure high-quality continuity of care
- Maintain professional certifications and adhere to facility guidelines, including infection control and documentation standards
Qualifications
- Current PTA credential (state license) — pending licenses accepted
- Minimum 1 year of skilled home health experience required
- BCLS certification required
- OASIS experience required; familiarity with EPIC and Rover strongly preferred
- Reliable transportation and ability to travel within an occasional 60-mile radius per day
- Strong communication skills and ability to work independently in patient homes
- First-time travel/contract clinicians are welcome
Education
- Associate degree from an accredited Physical Therapy Assistant program or equivalent; state PTA credential required or pending
Knowledge & Skills
- Proficient in home health clinical assessments and interventions
- Experienced with OASIS assessments and documentation standards
- Comfortable using EPIC remote client and Rover mobile charting
- Time management and route-planning skills to efficiently manage multiple visits
- Patient education, safety assessment, and mobility training
- Ability to meet productivity expectations while delivering quality care
