Roles & Responsibilities

• Provide compassionate hands-on care for residents, assisting with activities of daily living
• Monitor and report changes in resident condition to the nursing team promptly
• Document care accurately and thoroughly in the resident record
• Support safe mobility and fall-prevention strategies for each resident
• Collaborate with interdisciplinary team members to deliver person-centered care

Qualifications

• Current LNA certification (T-LNA designation noted for travel LNA assignments)
• Ability to pass facility onboarding, background check and any required screenings
• Comfort working evenings and committed to weekend coverage as scheduled
• Strong communication skills and a team-first attitude

Education

• Completion of a state-approved nursing assistant program or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Skilled in ADLs: bathing, dressing, toileting and grooming support
• Basic vital signs measurement and documentation
• Infection control and safe patient handling practices
• Ability to follow care plans and escalate concerns appropriately
• Time management and reliable attendance for scheduled shifts
