Roles & Responsibilities

• Conduct thorough screening, assessment, diagnosis, and treatment planning for patients from birth through elder
• Provide brief therapy and continuing care to individuals and families
• Lead and coordinate discharge planning, including ALF and transitional placements
• Facilitate or co-facilitate patient care conferences with multidisciplinary teams
• Advocate for clients and assist with complex matters such as Power of Attorney when relevant
• Coordinate care with hospital teams and community partners to ensure appropriate services and smooth transitions
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and billing standards
• Support quality management activities and participate in trainings to improve clinical practice
• Assist with billing duties and provider enrollment documentation as assigned

Qualifications

• Masters Degree in Social Work, Psychology, Counseling, or a related discipline
• Licensed or in the process of becoming licensed as LCSW, LPC, or LMFT
• Valid State of Alaska driver license or ability to obtain within six months of hire
• Must be 21 years of age
• Clinical Competency Assessment (SEARHC) required within 3 months of hire and every three years
• Comfort with discharge planning, care coordination, crisis intervention, and documentation standards

Education

• Masters Degree in Social Work, Psychology, Counseling or related discipline

Knowledge & Skills

• Discharge planning and care coordination best practices
• Clinical assessment, screening, diagnosis, and brief therapy techniques
• Crisis intervention and interviewing techniques
• Documentation and compliance with accreditation and billing requirements
• Case management and multidisciplinary team collaboration
• Chemical dependency support and mental health assessment
• Familiarity with tribal health and cultural wellness approaches
