Roles & Responsibilities

• Provide high-quality, patient-centered physical therapy care in the home setting
• Perform thorough assessments and develop individualized treatment plans
• Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs
• Document clinical findings and progress clearly and timely in the charting system
• Collaborate with interdisciplinary care team members to coordinate patient care
• Demonstrate professional judgment when evaluating home safety and equipment needs
• Participate in required competency testing and skills checklists to maintain clinical excellence

Qualifications

• Licensed Physical Therapist in good standing (license verification required)
• Current BLS certification
• Home health experience preferred; comfortable delivering care in patients' homes
• Epic experience preferred but not required
• Successful completion of Trinity specialty competency testing and skills checklist
• Ability to clear required health screenings and immunizations (drug screen, MMR, Hep B, COVID-19 vaccine, influenza vaccine, Tdap, 2-step PPD or IGRA, varicella)
• Fit testing and any required physical/health statement as part of onboarding

Knowledge & Skills

• Strong assessment and treatment planning skills for adult and geriatric populations
• Skilled in functional mobility training, gait training, and ADL-focused interventions
• Excellent patient education and communication skills with patients and families
• Proficient documentation skills; Epic experience is a plus
• Ability to work independently, prioritize caseloads, and manage time effectively
• Comfortable performing home safety evaluations and recommending assistive devices
