Roles & Responsibilities
- Assess patients regularly and deliver skilled nursing care following the physician’s plan of care
- Initiate, revise, and adapt individualized care plans to meet changing patient needs
- Provide medication management and assess therapeutic responses with precision
- Coordinate interdisciplinary care and communicate patient status to the care team
- Collaborate with CNAs and other staff to ensure consistent, compassionate care
- Notify physicians of condition changes and obtain new orders as needed
- Document care clearly and accurately in the EMR (Epic) to support quality outcomes
Qualifications
- Active Michigan RN license (LARA/Nursys verification required) — must be active
- 2+ years of Long Term Care nursing experience (required)
- Current American Heart Association BLS certification (required)
- Completed skills checklist (current within 1 year) relevant to LTC (required)
- Two supervisory reference forms completed within the last two years (required)
- Experience with Epic EMR preferred/required as noted
- Comfort coordinating care across the interdisciplinary team and mentoring CNAs
Knowledge & Skills
- Strong patient assessment and clinical judgment using the nursing process
- Care planning, documentation, and outcomes evaluation
- Medication administration and therapeutic response monitoring
- Care coordination across disciplines and effective communication
- Proficiency with Epic electronic medical records
- BLS (AHA) certified and up-to-date clinical skills for LTC populations
