Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans that improve function
- Deliver skilled therapy to enhance mobility, strength, balance, and independence
- Document evaluations, treatment sessions, and patient progress accurately and in a timely manner
- Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
- Educate patients and caregivers on therapeutic exercises, safety, and mobility strategies
- Maintain organized, safe, and compliant treatment areas
- Ensure adherence to facility policies, procedures, and state regulations
Qualifications
- Active California Physical Therapist (PT) license (required)
- Prior SNF or rehabilitation experience preferred
- Strong clinical judgment with excellent communication and documentation skills
- Ability to work independently and as part of a collaborative team
- Availability to commit to the full 13-week contract

- Improving mobility, strength, balance, and functional independence
- Accurate and timely clinical documentation
- Interdisciplinary collaboration and clear communication with team members
- Patient and caregiver education for home safety and therapeutic carryover
- Familiarity with facility policies and state regulatory requirements
