Case Manager - Acute Hospital
Location: Santa Rosa, CA
Start Date: 05-18-2026
End Date: 08-17-2026
Contract Type: Travel Contract
Shift Type: Primarily Monday-Friday, 8:00 AM - 4:30 PM (Day shift); 5x8 schedule with 2 weekends required every 6 weeks (weekend coverage rotates), occasional floating between like units as needed
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate patient care across the continuum by actively managing discharge planning and follow-up
- Conduct utilization review and apply InterQual and admission criteria to determine medical necessity
- Manage prior authorizations and arrange DME, SNF placements and ambulance transport independently
- Collaborate with physicians, nursing, social services and ancillary teams to develop and implement individualized plans of care
- Perform needs assessments and ensure benefits eligibility to support safe, timely discharges
- Document thoroughly and accurately in Epic to maintain continuity of care and compliance
- Support worker’s compensation case management and disability coordination when applicable
- Participate in family meetings and contribute to treatment plan development
- Abstract data and contribute to quality/regulatory initiatives (DRG, CMS, NCQA, Joint Commission standards)
- Serve as a patient advocate to optimize outcomes and reduce unnecessary length of stay
Qualifications
- Minimum 2 years acute hospital case management experience with hands-on care coordination and discharge planning
- Experience working in fast-paced acute care settings; travel experience required/preferred
- Proficiency with Epic charting system (required)
- BLS certification preferred
- Comfort managing high patient ratios (experience with ~1:20 noted)
- Demonstrated ability to use InterQual criteria, determine medical necessity, and complete prior authorizations
- Experience arranging DME, SNF placement and ambulance transport independently
- Strong communication skills and ability to lead family meetings and interdisciplinary care planning
- Willingness to float between like units based on department needs
- Able to meet clinician current work history requirements (within 2 years)
Knowledge & Skills
- Care coordination and discharge planning best practices
- Utilization review and InterQual application
- Prior authorization workflows and benefits eligibility
- DRG, CPT and ICD-10 coding awareness for case management and documentation
- Regulatory knowledge: CMS, NCQA, The Joint Commission, HIPAA and OSHA basics
- Worker’s compensation case management processes
- Data abstraction and reporting for quality improvement
- Familiarity with acute hospital specialties (ED, ICU, behavioral health, pediatrics, MS, NICU, SDU/PCU/IMC/Obs)
- Strong clinical judgment to determine admission criteria and plan of care
- Ability to coordinate resources: PT, RT, IV teams, interpretation services, social work and transport
