Roles & Responsibilities
- Provide high-quality physical therapy evaluations and individualized treatment plans for older adults
- Implement therapy interventions that improve mobility, safety, and residents’ quality of life
- Collaborate closely with the rehab team, nursing staff, and other healthcare partners to coordinate resident care
- Document therapy services accurately and promptly using Net Health
- Support and promote a resident-centered culture within the therapy department
- Communicate progress and care plans clearly with residents and families to encourage engagement
Qualifications
- PA Physical Therapist license in good standing
- CPR certification (American Heart Association or Red Cross)
- Two-step PPD/TB test completed within 30 days of start date
- Covid vaccination records or documented exemption
- Experience working in senior care or with older adult populations preferred
- Ability to complete background check and fingerprinting
- Current physical exam performed by a physician, PA, or nurse practitioner prior to start
- Resume on file and ready for review
Knowledge & Skills
- Patient evaluations and treatment planning tailored to older adults
- Skills in mobility improvement and safety-focused interventions
- Strong documentation practices (experience or willingness to use Net Health)
- Excellent communication and teamwork within interdisciplinary rehab teams
- Resident-centered care approach and compassionate bedside manner
- Ability to work across skilled nursing, personal care, and independent living settings
