Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on and therapeutic exercise interventions to improve mobility and function
• Collaborate with interdisciplinary team members to support resident-centered care
• Document skilled services clearly and timely in the facility electronic medical record
• Monitor progress, modify treatment plans as needed, and communicate outcomes to the team and family
• Support safe transfers, fall prevention, and functional independence for residents

Qualifications

• Current, unrestricted Physical Therapist license in Michigan (or eligible to obtain)
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting preferred
• Strong clinical reasoning and hands-on treatment skills
• Excellent documentation skills and familiarity with rehab-focused EMR systems
• Basic Life Support (BLS) certification preferred or willing to obtain
• Reliable, professional, and able to work collaboratively with nursing and rehab staff

Education

• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in assessment of balance, gait, mobility, and functional transfers
• Knowledge of restorative nursing and SNF care protocols
• Proficient with therapeutic exercise, manual therapy, and assistive device training
• Comfortable developing measurable goals and discharge plans
• Strong communication skills for resident care coordination and family updates
