Roles & Responsibilities
- Provide compassionate direct care to residents, assisting with activities of daily living
- Administer medications and perform treatments according to physician orders
- Monitor and document resident condition, vital signs, and clinical changes promptly
- Collaborate with interdisciplinary team members to create and follow individualized care plans
- Communicate effectively with residents and families to support education and discharge planning
- Maintain accurate electronic and paper charting in compliance with facility policies
- Adhere to infection control standards and safety protocols to protect residents and staff
Qualifications
- Active LPN license in Massachusetts (required)
- Completion of an accredited Practical Nursing program (LPN)
- BLS/CPR certification (required or willing to obtain upon hire)
- Experience in long-term care, rehab, or skilled nursing preferred
- Current immunizations and clear background check as required by facility
- Strong interpersonal skills and the ability to work collaboratively in a fast-paced environment
Education
- Graduate of an accredited Practical Nursing (LPN) program; LPN diploma or certificate
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and dressing changes
- Familiarity with electronic medical record (EMR) systems and clinical charting
- Skilled at taking and interpreting vital signs and recognizing clinical changes
- Excellent communication, resident advocacy, and teamwork abilities
- Knowledge of infection prevention and patient safety protocols
