Roles & Responsibilities

• Provide compassionate direct care to residents, assisting with activities of daily living
• Administer medications and perform treatments according to physician orders
• Monitor and document resident condition, vital signs, and clinical changes promptly
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Communicate effectively with residents and families to support education and discharge planning
• Maintain accurate electronic and paper charting in compliance with facility policies
• Adhere to infection control standards and safety protocols to protect residents and staff

Qualifications

• Active LPN license in Massachusetts (required)
• Completion of an accredited Practical Nursing program (LPN)
• BLS/CPR certification (required or willing to obtain upon hire)
• Experience in long-term care, rehab, or skilled nursing preferred
• Current immunizations and clear background check as required by facility
• Strong interpersonal skills and the ability to work collaboratively in a fast-paced environment

Education

• Graduate of an accredited Practical Nursing (LPN) program; LPN diploma or certificate

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and dressing changes
• Familiarity with electronic medical record (EMR) systems and clinical charting
• Skilled at taking and interpreting vital signs and recognizing clinical changes
• Excellent communication, resident advocacy, and teamwork abilities
• Knowledge of infection prevention and patient safety protocols
