Roles & Responsibilities
- Assess and evaluate residents’ communication and swallowing function using evidence-based tools
- Develop individualized treatment plans to improve speech, language, cognition, and safe swallowing
- Deliver skilled therapy in the SNF setting and adjust care based on progress and team input
- Collaborate closely with nurses, rehab colleagues, and families to support holistic patient goals
- Document thoroughly and timely to meet facility and regulatory standards
- Educate residents, families, and staff on strategies to carry over therapy gains into daily life
Qualifications
- ASHA Certificate of Clinical Competence (CCC) — listed as a primary skill for this role
- Speech-Language Pathologist (SLP) credential and clinical training in adult/SNF care
- Open to all experience levels — any experience accepted, including new graduates
- Strong interpersonal skills and comfort working as part of an interdisciplinary team
- Current basic professional requirements (immunizations, background check, etc.) as required by facility
Education
- Master's degree in Speech-Language Pathology (preferred)
Knowledge & Skills
- Proficiency in adult speech, language, cognitive-communication, and dysphagia assessment
- Skilled in developing goal-driven, functional treatment plans
- Strong documentation and clinical reasoning skills tailored to SNF settings
- Ability to collaborate with nursing and rehab staff to implement care plans
- Comfort with educating family members and caregivers on carryover strategies
