Roles & Responsibilities

• Provide compassionate, hands-on patient care that makes a real difference every shift
• Administer medications and treatments safely and accurately under facility protocols
• Perform routine assessments and document patient status changes clearly in the chart
• Collaborate with RNs, physicians, and the interdisciplinary care team to coordinate care
• Monitor and record vital signs and communicate concerns promptly
• Assist with admissions, transfers, and discharges to ensure smooth transitions
• Educate patients and families about care plans and self-care in a friendly, understandable way
• Maintain infection control and safety standards to protect patients and colleagues

Qualifications

• Active LPN license in Pennsylvania required
• Graduate of an accredited LPN/Vocational Nursing program
• Current BLS/CPR certification required
• Strong communication and teamwork skills — you’ll be part of a collaborative care team
• Comfort with electronic medical records and clear clinical documentation
• Previous experience in long-term care, skilled nursing, or similar settings is a plus

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and safe documentation
• Basic wound care and dressing changes
• Patient assessment and vital signs monitoring
• Familiarity with EMR/charting systems
• Infection control practices and safety protocols
• Time management, prioritization, and delegation within scope
• Compassionate patient and family communication
