Roles & Responsibilities

• Provide compassionate direct patient care, including assessments and routine treatments
• Administer medications and document treatments accurately and timely
• Collaborate with the interdisciplinary team to develop and update patient care plans
• Monitor patient status and escalate changes to RN/physician as needed
• Maintain clear, professional charting and electronic health record documentation
• Support safe, respectful transitions of care during shift handoffs and every other weekend coverage
• Adhere to facility infection control and safety policies to protect patients and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (New Jersey license preferred)
• BLS/CPR certification required; additional certifications welcomed
• Comfortable working the day shift with an every-other-weekend requirement
• Prior clinical experience preferred; experience in long-term care or rehab is a plus
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN/Vocational Nursing program
• Registered Nurse candidates should hold an ADN or BSN

Knowledge & Skills

• Solid knowledge of basic nursing assessments and vital sign monitoring
• Medication administration and documentation best practices
• Familiarity with infection prevention and wound care fundamentals
• Comfortable using electronic health record systems and documenting care
• Good time management, prioritization and teamwork skills
