Roles & Responsibilities
- Provide direct physical therapy services to home health clients, delivering person-centered care
- Collaborate with interdisciplinary team members to develop and update comprehensive plans of care
- Perform skilled assessments and document OASIS-compliant clinical findings and progress
- Prescribe and fit rehabilitation equipment (splints, orthoses, prostheses, wheelchairs, ADL aids) as appropriate
- Use therapeutic modalities and evidence-based interventions to help patients reach functional goals
- Educate patients and caregivers on safe home programs, fall prevention, and adaptive strategies
- Work independently in the field while maintaining clear, timely communication with the agency
Qualifications
- Home Health Certification (required) — demonstrated proficiency in home care practices
- Active BLS (AHA) certification (required)
- Valid driver’s license (required) for home visits
- Massachusetts state license (must be uploaded with profile as required)
- Minimum 2+ years combined acute hospital-based nursing/care and home care experience (as specified)
- Proficiency with OASIS documentation (required and non-negotiable)
- Ability to work independently with minimal supervision and excellent time management
Knowledge & Skills
- Strong clinical skills in physical therapy and rehabilitation
- Experience with OASIS assessments and home health documentation
- Familiarity with rehabilitation equipment and modalities
- Ability to develop and implement individualized care plans
- Solid communication and collaboration skills for interdisciplinary care
- Comfort working autonomously in patients’ homes and adapting care to varied environments
