Roles & Responsibilities

• Provide compassionate, patient-centered nursing care on the inpatient rehab unit
• Administer medications including IV medications, PICC line meds, PEG tube meds and blood products safely and accurately
• Perform specialized nursing procedures such as tracheostomy care, tube feedings, bladder scans, Foley catheter care and bladder irrigation
• Document care thoroughly in Meditech to ensure continuity and regulatory compliance
• Collaborate closely with physical therapy, occupational therapy, speech therapy, physiatry and the interdisciplinary team to support patient functional goals
• Monitor patient status, identify changes, and escalate concerns to providers to keep patients safe
• Support patient and family education around care plans and discharge goals to promote independence

Qualifications

• Active LPN or LVN license, Virginia nursing license required
• BLS certification required
• Minimum 2 years clinical nursing experience, Med/Surg or PCU experience required
• Rehab experience preferred
• Comfortable with IV medication administration, PICC and PEG tube care, and blood administration
• Ability to document in Meditech or quickly learn the charting system
• Willingness to float to other med-surg departments as needed

Education

• LPN/LVN diploma or equivalent, active state licensure required

Knowledge & Skills

• Solid knowledge of rehabilitation nursing principles and patient functional goal planning
• Skill with IV medication administration and working with Baxter IV pumps
• Familiarity with PICC, PEG tube administration and trach care
• Proficiency using Meditech for documentation
• Competence with bladder scanners, Foley catheter care and bladder irrigation
• Strong communication and teamwork skills in a multidisciplinary environment
• Ability to manage patient ratios typical for the unit and prioritize care effectively
