Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as an LPN, focusing on safety and comfort
• Administer medications and treatments accurately and document per facility policy
• Monitor patient condition, report changes to the RN or provider, and participate in care planning
• Collaborate with interdisciplinary team members to deliver coordinated, person-centered care
• Maintain clear, timely charting and ensure chart accuracy to support continuity of care
• Support resident and family education with patience and empathy

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico or eligibility to obtain prior to start
• Recent clinical experience as an LPN in long-term care or similar setting preferred
• Current Basic Life Support (BLS) certification or willingness to obtain per facility requirements
• Ability to work overnight shifts and adapt to a travel assignment schedule
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Reliable documentation practices and familiarity with electronic charting
• Basic patient assessment and vital signs monitoring
• Infection control best practices and adherence to facility protocols
• Patient-centered communication and teamwork
