Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatments in patients' homes to improve function and independence
• Create and implement individualized treatment plans based on thorough assessments
• Document clinical care accurately and promptly using HCHB and other required systems
• Coordinate care with nurses, hospice (when applicable), and other members of the home health team
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Manage a home health caseload efficiently while maintaining high-quality, compassionate care
• Maintain reliable transportation and plan patient visits for timely, professional service

Qualifications

• Minimum 2 years of clinical experience (as listed in the posting)
• Home health experience required for therapy positions (explicit requirement)
• Comfortable and experienced with patient documentation and HCHB
• Reliable vehicle for patient visits and willingness to travel within the assigned county/region
• Ability to work Monday through Friday daytime schedule

Knowledge & Skills

• Strong home health clinical skills and familiarity with patient-centered home assessments
• Proficient patient documentation practices, especially in HCHB
• Experience scheduling and coordinating 5-6 patient visits per day
• Excellent communication and patient education skills
• Time management and caseload prioritization for efficient home visits
