Roles & Responsibilities
- Conduct comprehensive screening, clinical assessments, and diagnostic evaluations across the lifespan
- Develop and implement treatment plans and provide brief therapy and continuing care
- Lead and facilitate discharge planning and patient care conferences, including transitional/ALF placements
- Coordinate care with hospital teams and community partners to ensure smooth transitions and optimal outcomes
- Provide client advocacy, skill-building, and assistance with complex matters such as Power of Attorney when relevant
- Document care promptly and accurately to meet state, federal, accreditation, and billing standards
- Support chemical dependency and mental health referrals and interventions as needed
- Participate in quality management, trainings, and multidisciplinary team meetings to improve services
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or a related discipline from an accredited institution
- Licensed (or in the process of becoming licensed) as an LCSW, LPC, or LMFT
- Valid Alaska driver’s license or able to obtain one within 6 months of hire
- Must be 21 years of age
- Clinical competency required within 3 months of hire and ongoing every three years
- Experience or comfort working with Tribal health systems and in rural/critical access hospital settings is a plus
Education
- Master's Degree in Social Work, Psychology, Counseling, or related discipline
Knowledge & Skills
- Expertise in discharge planning and care coordination
- Strong skills in interviewing, crisis intervention, and brief therapy
- Thorough knowledge of documentation regulations, accreditation standards, and patient legal rights
- Ability to perform case management, analyze and diagnose clinical situations, and adopt effective courses of action
- Clear, concise clinical writing and charting in accordance with professional and state standards
- Collaborative teamwork and effective communication with multidisciplinary teams and community partners
