Roles & Responsibilities

• Conduct thorough psychosocial assessments to understand each patient’s unique needs
• Develop individualized, evidence-based treatment plans that promote lasting recovery
• Deliver individual and family therapy across the lifespan using proven interventions
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues for coordinated whole-person care
• Coordinate care and referrals to ensure patients access the right community resources
• Maintain timely, accurate clinical documentation to support quality care and outcomes
• Contribute to program development and quality improvement initiatives focused on rural and underserved populations

Qualifications

• Active LISW license in Iowa (required)
• Master’s degree in Social Work from an accredited program (required)
• Minimum of 2 years of clinical experience preferred
• Experience working in rural or medically underserved settings is a plus
• Strong interpersonal and organizational skills with excellent clinical documentation abilities

• Master’s degree in Social Work from an accredited program
