Roles & Responsibilities

• Evaluate patients to identify movement dysfunction, pain sources, and rehab needs
• Develop and implement treatment plans that reduce fall risk and improve mobility
• Provide mobility training and therapeutic interventions to enhance independence
• Collaborate on discharge planning to ensure safe transitions and positive outcomes
• Document all patient care thoroughly — evaluations, progress notes, discharges and daily notes
• Manage communications with providers, insurance companies, patients and rehab staff
• Handle insurance approvals/denials and document phone conversations related to care
• Support clinic upkeep by assisting with cleaning, laundry, and treatment room readiness
• Perform other duties as requested to keep patient care running smoothly

Qualifications

• Current Physical Therapist license in the state of Wisconsin or eligibility to obtain one
• Basic Life Support (BLS/CPR) certification (or willingness to obtain upon hire)
• Comfortable documenting evaluations, progress notes, discharges and daily notes
• Strong communication skills for collaborating with providers, patients and payers
• Prior clinical experience in outpatient, acute, or rehab settings preferred
• Ability to assist with light facility tasks (treatment room cleaning, laundry) as part of the team

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred; equivalent clinical training considered

Knowledge & Skills

• Patient evaluation and clinical reasoning to guide effective treatment plans
• Treatment planning and implementation for movement dysfunction and pain management
• Mobility training techniques to improve functional independence
• Discharge planning and coordination with interdisciplinary teams
• Thorough clinical documentation and familiarity with insurance communication
• Strong interpersonal skills for patient-centered care and teamwork
