Roles & Responsibilities

• Assess patient mobility, function and pain levels to build meaningful treatment plans
• Develop individualized therapy goals and implement evidence-based interventions
• Deliver hands-on and restorative care focused on safe, functional outcomes
• Collaborate with nursing, physicians and therapy team members to coordinate care
• Educate patients and families on home exercise programs and safe transfer techniques
• Document progress clearly and timely in the facility EHR to support continuity of care
• Participate in team meetings and provide clinical recommendations to improve patient outcomes

Qualifications

• Active or eligible Pennsylvania Physical Therapist license
• Graduation from an accredited Physical Therapy program and current CPR/BLS certification
• Experience in skilled nursing, outpatient, or post-acute settings preferred
• Strong clinical assessment and manual therapy skills
• Excellent communication and teamwork skills and the ability to work independently
• Ability to pass facility background checks and required onboarding screenings

Education

• Doctor of Physical Therapy (DPT) preferred or Master of Physical Therapy from an accredited program

Knowledge & Skills

• Skilled in patient evaluation, treatment planning and therapeutic exercise
• Knowledge of mobility training, fall prevention and safe transfer techniques
• Familiarity with SNF standards of care and documentation best practices
• Comfortable using common EHR systems and completing timely clinical notes
• Ability to prioritize caseloads and adapt treatments to individual patient needs
• Strong interpersonal skills for patient and family education
