Roles & Responsibilities

• Interpret and accurately transcribe medication orders into the electronic medication profile
• Prepare, compound and dispense pharmaceuticals, including IV admixtures, chemotherapy and parenteral nutrition
• Manage controlled substances and maintain legally required records
• Monitor drug therapy for interactions, allergies and appropriate dosing, and perform pharmacokinetic dosing per protocol
• Review culture & sensitivity data to guide antibiotic stewardship and recommend therapy adjustments
• Provide clinical consultation and drug information to providers, patients and families
• Support formulary management by promoting therapeutic substitution and cost-effective medication choices
• Document clinical activities, participate in quality monitoring, and contribute to continuous improvement
• Serve as a member of the Code Response Team to support emergent patient care

Qualifications

• Active CO PHA (Pharmacist) license in good standing
• BS Pharmacy from an ACPE-accredited school (required)
• Epic experience REQUIRED (facility converting from Meditech to Epic)
• BLS (American Heart Association) current card
• ACLS (American Heart Association) current card
• Flu vaccine required (Oct 1 through Apr 1; no exemptions accepted)
• Meditech experience/training is a plus
• Traveler-ready documentation: resume, skill sheet, proof of flu vaccine, BLS/ACLS cards and CO PHA license

Education

• BS Pharmacy (ACPE-accredited)

• Medication order interpretation and computerized medication profile management
• IV admixture compounding, chemotherapy preparation and parenteral nutrition
• Pharmacokinetic dosing and therapeutic drug monitoring
• Antibiotic stewardship with C&S review and therapy recommendation
• Controlled substances management and regulatory compliance
• Formulary management and therapeutic substitution
• Clinical consultation, drug information delivery and patient education
• Thorough documentation, quality monitoring and participation in safety initiatives
• Ability to function as part of a Code Response Team
