Roles & Responsibilities
- Provide compassionate, hands-on nursing care including administering medications and monitoring vital signs
- Perform wound care and treatments as ordered to support resident recovery
- Document resident conditions and care accurately and in a timely manner
- Collaborate with RNs, CNAs, and the interdisciplinary team to implement and update care plans
- Assist residents with activities of daily living while promoting independence and dignity
- Recognize changes in resident status and escalate concerns to the appropriate team members
- Support admissions, transfers, and discharges to ensure smooth transitions of care
Qualifications
- Current Licensed Practical Nurse (LPN) license in Massachusetts (required)
- BLS/CPR certification preferred
- Previous experience in long-term care or skilled nursing is a plus
- Strong communication skills and the ability to work well on a team
- Willingness to complete facility-required background checks and onboarding
Education
- Graduate of an accredited practical nursing (LPN) program
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and treatment follow-through
- Electronic charting / EHR proficiency
- Resident-focused care and ability to prioritize tasks
- Clear communication with residents, families, and clinical teams
- Time management and dependable clinical judgment
