Roles & Responsibilities
- Facilitate, coordinate, and lead the Long-Term Care QAPI program to drive measurable improvements
- Plan and direct quality assurance activities, integrating QAPI into everyday operations
- Conduct chart audits, review clinical outcomes and identify opportunities for improvement
- Coordinate regulatory surveys and maintain accreditation readiness across the facility
- Lead root cause analyses (RCA) and develop actionable plans to address deficiencies
- Track recommendations, maintain meeting minutes, and report progress to leadership
- Collaborate with Administration, Providers, and interdisciplinary teams to enhance patient safety
- Support incident investigations and maintain accurate records in Datix and other systems
- Design, implement, and evaluate education and coaching for staff to improve clinical practices
- Participate in facility surveys and contribute to policy/procedure review and updates
Qualifications
- Current, full, active Registered Nurse license in Alaska (or applied for prior to start for direct hire)
- Certified Professional in Healthcare Quality (CPHQ) or QAPI Certified Professional (QCP) or equivalent LTC quality certification obtained within 3 years
- Basic Life Support (BLS) — American Heart Association
- Clinical Competency Assessment (SEARHC) or equivalent
- Minimum 2 years of experience monitoring quality and regulatory requirements, planning and coordinating process changes
- Demonstrated leadership in facilitating cross-functional teams and collaborative improvement efforts
- Preferred: experience working in Long Term Care and familiarity with CMS regulatory standards
- Ability and willingness to travel approximately 30% of the time
Knowledge & Skills
- QAPI and quality assurance program design and execution
- Regulatory compliance (CMS) and accreditation readiness across long-term care
- Infection control principles and patient safety practices
- Medical chart review and data extraction for quality measurement
- Data analytics: collecting, analyzing and presenting clinical outcome data
- Proficiency with Microsoft Office (Word, Excel, PowerPoint) and Datix event reporting
- Root Cause Analysis (RCA) facilitation and corrective action planning
- Strong verbal and written communication, coaching and meeting facilitation
- Ability to prioritize, problem-solve, and drive performance improvement
