Roles & Responsibilities
- Provide compassionate direct care to PACE participants, including medication administration, basic wound care, and assistance with activities of daily living
- Perform accurate assessments and document changes in participant condition in the medical record
- Collaborate with an interdisciplinary team to support individualized care plans and coordinate participant needs
- Educate participants and families on care plans, safety measures, and health maintenance
- Follow infection control protocols, complete required screenings and maintain up-to-date immunizations
- Complete Trinity Skills Checklist and participate in competency evaluations to ensure high-quality care
Qualifications
- Active, verifiable LPN license (License/Certificate Verification required)
- Current Basic Life Support (BLS) certification
- Completion of Trinity Skills Checklist and successful credentialing
- Required health screenings and immunizations: Fit Test, 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 and Influenza vaccines, Trinity 10 Panel Drug Screen, and a Physical/Health Statement
- Comfortable working in a per diem/pool model with flexible availability
- Excellent communication, organization, and teamwork skills
Education
- Completion of an accredited Practical Nursing program
- Diploma or certificate qualifying for LPN licensure
Knowledge & Skills
- Patient assessment and monitoring (vital signs, basic clinical observations)
- Medication administration and documentation
- Basic wound care and ADL support
- Infection prevention and control practices
- Electronic health record documentation and care coordination
- Understanding of geriatric care needs and chronic disease management
- Strong interpersonal skills for working with participants, families, and the care team
