Roles & Responsibilities

• Provide skilled occupational therapy evaluations and evidence-based treatment plans tailored to each patient
• Deliver individualized ADL/IADL training and adaptive equipment recommendations to maximize independence
• Collaborate with nursing, PT, SLP and the interdisciplinary team to coordinate seamless patient care
• Document progress clearly and timely in the patient record and communicate changes to the care team
• Educate patients and families on home programs, safety, and strategies to support long-term outcomes
• Participate in team meetings and discharge planning to ensure safe transitions of care

Qualifications

• Current, unrestricted Occupational Therapy license (state licensure required; ability to obtain OH license if not already held)
• Active NBCOT certification preferred (or equivalent credential)
• Current BLS/CPR certification required or willingness to obtain prior to start
• Experience in skilled nursing, inpatient rehab, or post-acute care settings preferred
• Ability to pass background check and meet facility immunization requirements
• Strong clinical reasoning, documentation skills, and ability to work independently

Education

• Bachelor's or Master's degree in Occupational Therapy (or OTD/MSOT preferred)

Knowledge & Skills

• Skilled in OT assessment, treatment planning, and ADL retraining
• Familiarity with adaptive equipment and home safety recommendations
• Proficient documentation habits and comfortable with electronic health records
• Strong interpersonal and patient education skills
• Ability to work well within an interdisciplinary team and communicate effectively
