Roles & Responsibilities

• Conduct clinical screening, assessment, diagnosis, treatment planning and brief therapy across the lifespan
• Lead and coordinate inpatient discharge planning, including ALF and transitional placement arrangements
• Provide care coordination and client advocacy to ensure smooth transitions across the continuum of care
• Deliver clinical support for chemical dependency and mental health needs and make appropriate community referrals
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and billing standards
• Facilitate or co-facilitate patient care conferences and collaborate with hospital teams and community partners
• Complete provider enrollment and billing duties as assigned and contribute to quality management activities
• Participate in staff trainings and integrate new practices into client service delivery

Qualifications

• Master's degree in Social Work, Psychology, Counseling or a related discipline
• Licensed as or in the process of becoming an LCSW, LPC, or LMFT
• Valid State of Alaska driver’s license or ability to obtain within 6 months
• Must be 21 years of age
• Clinical competency assessment (SEARHC) required within 3 months of hire and every three years
• Ability to complete timely documentation, meet accreditation and billing requirements, and handle sensitive information

Education

• Master's Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution

Knowledge & Skills

• Discharge planning and care coordination across the continuum
• Screening, assessment, diagnosis, treatment planning and brief therapy
• Interviewing techniques, crisis intervention and ethical/legal patient management
• Documentation regulations specific to accreditation and hospital billing standards
• Case management, patient advocacy, and working with patients of all ages
• Familiarity with chemical dependency/substance use support and tribal/native health practices
