Roles & Responsibilities

Deliver comprehensive outpatient family medicine care across the lifespan with compassion and clinical excellence
Build lasting, trust-based relationships with patients and their families to support long-term health
Collaborate closely with experienced clinical and administrative staff to coordinate care
Participate in community-focused efforts to improve local health outcomes
Document patient encounters accurately and contribute to quality improvement initiatives

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for New York State medical licensure
DEA certification or eligibility
Passion for community-focused, patient-centered primary care

Education

MD or DO degree from an accredited medical school

Knowledge & Skills

Outpatient primary care and family medicine across the lifespan
Patient-centered care and strong relationship-building abilities
Clinical collaboration within interprofessional teams
Experience delivering high-quality preventive and chronic disease management
Comfort practicing in a community/rural health setting
