Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and craft individualized treatment plans
• Deliver targeted physical therapy interventions to improve strength, mobility, and functional independence
• Educate patients and families on exercises, safe movement techniques, and home modifications to support recovery
• Collaborate with occupational therapists, nurses, physicians, and the wider care team to coordinate seamless care
• Monitor patient progress, adjust treatment plans as needed, and document outcomes clearly and timely
• Ensure care plans comply with regulatory standards and facility policies while advocating for best practices

Qualifications

• Valid Physical Therapy license in the state of California (or ability to obtain prior to start)
• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Experience working in a skilled nursing facility or similar post-acute setting preferred
• Strong communication, interpersonal, and problem-solving skills
• Demonstrated ability to work independently and as part of a collaborative interdisciplinary team
• Compassionate, patient-focused approach with excellent documentation practices

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program

Knowledge & Skills

• Patient assessment and rehabilitation treatment planning
• Therapeutic interventions and use of therapeutic modalities
• Familiarity with adaptive equipment and fall prevention strategies
• Accurate clinical documentation and regulatory compliance
• Interdisciplinary collaboration and care coordination
• Patient and family education techniques
