Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in accordance with facility policies
• Perform baseline and ongoing assessments and document changes in condition
• Administer medications and treatments safely and accurately
• Collaborate with interdisciplinary team members to develop and update care plans
• Complete competency-based tasks and Trinity specialty checks as required
• Maintain accurate, timely documentation in the health record
• Adhere to infection control practices and all required health screenings

Qualifications

• Current LPN license (practical/vocational nursing) in Massachusetts or eligible to obtain state licensure
• BLS (Basic Life Support) certification required
• Willingness and ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Successful completion of required health screenings: fit test, respirator evaluation, 10-panel drug screen, TB testing (2-step PPD or IGRA), and current immunizations (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
• Ability to pass license/certificate verification and background checks
• Strong communication skills and a commitment to resident dignity and safety
• Prior experience in long-term care or skilled nursing preferred

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital sign monitoring
• Familiarity with infection control principles and PPE use
• Comfort with electronic health records and timely charting
• Ability to prioritize tasks in a dynamic, multi-patient environment
• Teamwork with nursing and interdisciplinary staff to support quality care
• Competency with specialty procedures as validated by Trinity testing
