Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in accordance with facility policies
- Perform baseline and ongoing assessments and document changes in condition
- Administer medications and treatments safely and accurately
- Collaborate with interdisciplinary team members to develop and update care plans
- Complete competency-based tasks and Trinity specialty checks as required
- Maintain accurate, timely documentation in the health record
- Adhere to infection control practices and all required health screenings
Qualifications
- Current LPN license (practical/vocational nursing) in Massachusetts or eligible to obtain state licensure
- BLS (Basic Life Support) certification required
- Willingness and ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Successful completion of required health screenings: fit test, respirator evaluation, 10-panel drug screen, TB testing (2-step PPD or IGRA), and current immunizations (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Ability to pass license/certificate verification and background checks
- Strong communication skills and a commitment to resident dignity and safety
- Prior experience in long-term care or skilled nursing preferred
Education
- Graduate of an accredited Practical Nursing (LPN/LVN) program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic patient assessment and vital sign monitoring
- Familiarity with infection control principles and PPE use
- Comfort with electronic health records and timely charting
- Ability to prioritize tasks in a dynamic, multi-patient environment
- Teamwork with nursing and interdisciplinary staff to support quality care
- Competency with specialty procedures as validated by Trinity testing
