Roles & Responsibilities

• Perform thorough functional evaluations of SNF patients to determine therapy needs
• Develop and implement purposeful, patient-centered treatment plans focused on ADLs
• Provide therapeutic interventions that maximize patients' physical and mental functioning
• Document patient progress and modify plans of care based on outcomes
• Collaborate with interdisciplinary team members to coordinate care and discharge planning
• Provide coverage and services at partner sites such as Maluhia as needed

Qualifications

• Licensed Occupational Therapist (OT) in Hawaii or eligible for Hawaii licensure
• Current state-required credentials and ability to pass facility onboarding
• Experience performing functional evaluations and treatment planning in SNF or similar settings preferred
• Strong patient assessment and ADL-focused intervention skills
• Excellent communication and teamwork skills

Education

• Degree in Occupational Therapy (Bachelor's, Master's, or OTD) required

Knowledge & Skills

• Skilled in functional evaluation and treatment planning
• Proficient with Activities of Daily Living (ADL) interventions
• Experience working in Skilled Nursing Facility (SNF) environments
• Ability to create meaningful, purposeful activity programs
• Strong clinical documentation and progress tracking
