Roles & Responsibilities

• Provide skilled physical therapy evaluations and individualized treatment plans in a home health setting
• Deliver therapeutic interventions that improve mobility, strength, and independence
• Document patient assessments, interventions, and progress using computerized charting systems
• Collaborate with patients, families, and the interdisciplinary team to coordinate high-quality care
• Monitor outcomes and adjust care plans to meet patient goals and safety needs
• Complete required orientation and training hours and maintain required certifications

Qualifications

• Current Physical Therapist license (License to Submit: Yes)
• BLS certification (required)
• Minimum 1 year of clinical experience
• Experience in Home Health or post-acute care (as listed)
• Comfortable with computerized charting and electronic documentation
• Willingness to work day shifts with rotating weekends and holiday coverage

Knowledge & Skills

• Physical therapy assessment and treatment techniques
• Home health care practices and post-acute rehabilitation
• Basic Life Support (BLS)
• Computerized charting and electronic medical records
• Strong communication, time management, and documentation skills
