Roles & Responsibilities

• Provide compassionate direct resident care in a long-term care setting, ensuring dignity and comfort
• Administer medications and treatments safely and accurately during 12-hour day shifts
• Perform daily assessments and document resident status thoroughly in PCC
• Collaborate with RNs and interdisciplinary team members to coordinate resident care plans
• Float between two floors as needed, maintaining continuity and quality of care across units
• Communicate clearly with residents and families, keeping them informed and involved
• Support infection prevention and safety protocols to protect residents and staff
• Contribute to a positive care environment and help uphold facility standards

Qualifications

• Active Licensed Practical Nurse (LPN) license in Wyoming or eligible to obtain one
• Experience or comfort working in long-term care settings; LTC experience is a strong plus
• Familiarity with PCC documentation and electronic charting preferred
• Ability to work 12-hour day shifts, float between floors, and adapt to unit needs
• Strong communication, teamwork, and resident-centered care mindset
• Current BLS/CPR certification preferred or willingness to obtain

Education

• Completion of an accredited LPN/LVN program or equivalent vocational nursing diploma

Knowledge & Skills

• Skilled in medication administration and routine skilled nursing tasks
• Knowledge of long-term care best practices and resident-focused care
• Proficient with PCC or similar electronic health record systems
• Good clinical judgment, assessment skills, and time management
• Ability to document accurately and communicate effectively with team and families
• Comfortable working across multiple units and adapting to changing priorities
