Roles & Responsibilities

• Assess and evaluate patients in their home environment to create personalized rehabilitation plans
• Develop and implement skilled physical therapy treatment plans focused on mobility, safety, and independence
• Document all care accurately and timely in EPIC to ensure continuity and quality of care
• Collaborate with nurses, aides, and care coordinators to deliver coordinated home health services
• Educate patients and family/caregivers on home exercise programs, fall prevention, and adaptive strategies
• Maintain compliance with required screenings, checklists, and facility policies for safe practice

Qualifications

• Active licensure/certification verification required (state license and credentials must be current)
• BLS certification required
• Experience or comfort working in Home Health settings is preferred
• Familiarity with EPIC or similar electronic medical records is a plus
• Willingness to complete Trinity-required testing and checklists (skills checklist, specialty competency testing)
• Up-to-date immunizations and health screenings as listed (COVID-19, MMR, Hep B, Tdap, Influenza, Varicella, TB screening, drug screen, respirator/fit test)

Knowledge & Skills

• Skilled in Physical Therapy assessment and treatment in home care environments
• Proficient with EPIC documentation or similar EMR systems
• Strong patient-centered care and teaching skills
• Knowledge of home safety evaluation, mobility and transfer training, and fall prevention
• Familiar with infection control practices and required health screenings
• Comfortable working independently and coordinating care across a home health team
