Roles & Responsibilities
- Conduct thorough clinical screening, assessment, diagnosis, and treatment planning for patients from birth through elder
- Provide brief therapy and continuing care tailored to individual and family needs
- Lead and co-facilitate patient care conferences and manage discharge planning, including ALF/transitional placements
- Coordinate care with hospital teams and local community partners to ensure smooth transitions and optimal outcomes
- Advocate for clients, assist with complex matters (e.g., Power of Attorney), and support skill-building
- Maintain accurate, timely, and compliant documentation aligned with state, federal, accreditation, and hospital billing standards
- Support chemical dependency and mental health needs with appropriate referrals and clinical interventions
- Participate in quality management trainings and incorporate best practices into client services
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or a related discipline from an accredited institution
- Licensed as (or actively in the process of becoming) an LCSW, LPC, or LMFT
- Clinical Competency Assessment (SEARHC) required within 3 months of hire and every three years
- Valid State of Alaska driver’s license or ability to obtain within 6 months
- Must be 21 years of age
- Ability to meet requirements for covered positions under the Indian Child Protection Act (ICPA) and any required background checks
Education
- Master's Degree in Social Work, Psychology, Counseling, or related discipline (accredited)
Knowledge & Skills
- Discharge planning and care coordination across the continuum of care
- Clinical assessment, screening, diagnosis, and treatment planning
- Brief therapy techniques and continuing care practices
- Strong documentation skills and knowledge of accreditation and billing documentation requirements
- Case management and client advocacy, including Power of Attorney assistance when needed
- Crisis intervention and interviewing techniques
- Experience with chemical dependency support and multidisciplinary team facilitation
- Familiarity with provider enrollment documentation and quality management practices
