Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatment plans in patients' homes
• Deliver patient-centered mobility, balance, and functional training to improve independence
• Document encounters accurately and timely using facility/charting systems (Epic preferred)
• Collaborate with interdisciplinary teams and families to coordinate safe discharge plans
• Educate patients and caregivers on home exercise programs and fall prevention
• Participate in required competency testing and maintain mandatory health screenings

Qualifications

• Current Physical Therapy license (license/certificate verification required)
• Basic Life Support (BLS) certification
• Home health experience and comfort working independently in patients' homes
• Epic experience preferred but not required
• Completion of Trinity Specialty Specific Competency Testing and Skills Checklist as required
• Up-to-date immunizations and successful completion of required health screenings (fit test, drug screen, TB testing, MMR, Varicella, Tdap, COVID-19, influenza)

Knowledge & Skills

• Strong home health assessment and clinical decision-making skills
• Excellent patient education and caregiver training capability
• Proficient documentation and charting practices; Epic familiarity is a plus
• Safe mobility, transfers, gait training, and fall-risk mitigation techniques
• Time management and routing skills to maximize daily caseload efficiency
• Professional communication with patients, families, and interdisciplinary teams
