Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments safely and accurately
• Monitor and document residents' vital signs and clinical changes
• Collaborate with the care team to update and follow individualized care plans
• Communicate clearly and courteously with residents and their families
• Maintain accurate electronic health record documentation
• Assist with wound care, ADLs and nursing interventions as needed
• Respond promptly to resident concerns and support a safe environment

Qualifications

• Current LPN license in good standing (required)
• BLS/CPR certification or willingness to obtain
• Experience in long-term care preferred but new-to-LTC LPNs who demonstrate strong fundamentals are welcome
• Ability to pass background check and drug screening
• Reliable transportation to the Troy facility and flexibility for per diem shifts

Education

• Practical Nursing diploma or equivalent; active LPN license

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and pressure injury prevention
• Infection prevention and control procedures
• Familiarity with resident-centered long-term care and ADL support
• Comfortable using electronic health record systems
• Strong communication, teamwork and time management skills
